New Patient Registration
To register with the practice you need to fill in a GMS1 form which can be downloaded here. or picked up from reception at any of the surgeries. You will then need an appointment with one of the healthcare assistants for a new patient check, which involves checking you medical history and medication.
You will need to bring with you your current passport, a utility bill or other proof of address within our area, and your National Insurance card/NHS number.
